At Hickman Family Dental, we work hard to ensure our patients have all the resources necessary to come to their appointment prepared and informed. Below, we have put together all the relevant documents so our patients are ready for their appointment.

New Patient

HIPAA

The Health Insurance Portability and Accountability Act (HIPAA) protects both patients and doctors. Thanks to this regulation, your medical information is limited to you, your doctor, and only people you deem appropriate. If there is anyone you wish to allow us to share information with, such as a parent or spouse, please fill out the forms below.

Payment Options

We accept cash, check, VISA, Mastercard, and Discover. Payment is expected as services are rendered unless prior financial arrangements have been made. *Accounts 90 days past due may incur a finance charge.
